(Monday-Friday 8am-5pm)No On Call
Job Description:
Responsibilities:
Lead MDS Assessments: Coordinate, complete, and submit accurate clinical assessments and documentation for Medicare and Medicaid residents.
Drive Reimbursement Accuracy: Ensure proper reimbursement through timely MDS data and compliance with regulatory standards.
Collaborate on Care Plans: Work closely with our interdisciplinary team to develop individualized care plans for each resident.
Ensure Regulatory Compliance: Stay current with evolving healthcare regulations and maintain documentation to meet federal and state standards.
Qualifications:
Graduate of an accredited nursing program
Current Texas nursing license (LVN or RN)
Valid Texas driver’s license
At least 1 year of clinical experience as an RN or LVN
Strong skills in communication, organization, time management, and documentation
Fluent in English (spoken and written), as required for medical documentation
